Status report - notes and observations by the Investigator.
Date & time of each unscheduled visit.
Audio tape recorded conversation with the patient if possible. (patients approval or guardian required).

Our Mission
To improve the standard of care provided to Aids Patients, Elderly, Disabled and Seniors.
Care Providers, Doctors and Nurses perform better when placed under a watchful eye.
Patients neglected by the staff who receive visits from Private Investigators get more attention. For visits involving seniors home alone, we'll also interview with neighbors or other interested person living in the area if requested to do so by the family.
